About Us:

FormConnections, Inc. is a software development company that focuses on building apps that allow you go paperless. We currently sell three FormConnect apps through Apple's App Store (FormConnect Pro, FormConnect Pro+, and FormConnect ProMax). These apps allow you to create digital forms for collecting data. FormConnect ProMax is also available on additional platforms including PC and Android for data collection.

Information Covered By This Privacy Policy:

This privacy policy provides information on how the FormConnect Pro, FormConnect Pro+, and FormConnect ProMax apps collect and store data. It also covers the FormConnections website (formconnections.com) and FormConnect ProMax online accounts and cloud sync where those services are offered.

Information Collected Using the FormConnect Apps:

When you use FormConnect Pro, FormConnect Pro+, or FormConnect ProMax, any data collected is stored locally on the device and is only accessible by the owner of that device. FormConnections, Inc. does not have access to this data.

If you choose to backup your data or sync your forms between your devices using Apple's iCloud solution (FormConnect Pro+), FormConnections, Inc. does not have access to this data. FormConnect ProMax server-backed sync is governed by your organization's account settings; FormConnections, Inc. does not use form field contents for marketing.

How does the App Access, Use, Store and Share Data

The FormConnect apps (FormConnect Pro, FormConnect Pro+, and FormConnect ProMax) may request access to Google, Dropbox, Box and OneDrive’s data for the purpose of displaying it to the user from within our app. The FormConnect apps require access to the user’s profile and home directory. The user profile is for displaying the user’s name and email address on the document export screen. Access to the home directory is so we can display the directory structure and allow the user to specify where they want to upload their documents.

Information That Can Be Shared Using The FormConnect Apps:

If the user chooses, he or she can export the data collected via the FormConnect apps into a file format (pdf, csv, xml, html, fmc) that can be shared with other users. This data can be transmitted via email or uploaded to a cloud file service such as iCloud, Dropbox, Box, Google Drive or Microsoft OneDrive. During the exporting of this data, FormConnections, Inc. does not have access to this data.

ProMax Online Accounts and This Website:

If you create a FormConnect ProMax online account or use cloud sync, FormConnections, Inc. stores the information needed to run that service: for example your name, email, country, subscription status, forms and records you save to the cloud, and how you set up teams and sharing. Payments are processed by Paddle; we do not store your full card details on our servers. We also keep basic server logs for security and support. On formconnections.com and the ProMax web app, optional analytics cookies are used only if you accept them in the cookie banner.

We use this data to operate your account, bill your subscription, sync your forms, and keep the service secure. We do not use the contents of your forms for our own marketing (as noted above for server-backed sync).

How Long We Keep Data and Who We Share It With:

We retain ProMax online account and cloud data while your subscription is active. If the account owner requests deletion in My Profile, access ends right away; we email a JSON and CSV copy of personal data, then delete account data after the number of days shown in the deletion confirmation (except records we must keep for legal, tax, or security reasons). Cookie choices on this site are remembered in your browser for about six months.

We do not sell personal data. We work with Paddle (billing), our cloud hosting and email providers, and—if you opt in—analytics or diagnostic tools. Some of these companies may process data in the United States or other countries. We do not share your form contents with them for their own marketing.

Your Rights:

You can update your profile in the ProMax web app. Account owners in Europe or the UK can request a JSON or CSV export from My Profile; we send it by email. The account owner can also request account deletion from My Profile. To change optional cookies, use the banner on formconnections.com or the Cookies button on ProMax login, registration, forgot-password, and public form pages.

For access, correction, deletion, or other privacy requests—or if you are not the account owner—email privacy@formconnections.com.

GDPR Compliance:

Under the EU GDPR and UK data protection rules, roles depend on how you use FormConnect. For data that stays on your device or in your iCloud account, you control what is collected. For ProMax online, FormConnections, Inc. is the data controller for your account and billing-related information. For personal data your forms collect about other people, you are the controller and we act as a processor when that data is stored in ProMax cloud, as described in our Data Processing Agreement for accounts that require it. You must obtain any consent needed from your own customers before collecting their data.
